The Burdett National Transition Nursing Network

A collaborative initiative to improve the transition experience for young people with long-term health conditions from children to adult services.

The network focused on enhancing the quality of care and support during this critical period, which is essential for the long-term health and well-being of young people. The network’s approach included the implementation of a Model of Improvement for Transition, which had been developed at Leeds Teaching Hospital Trust.

This model creates a seamless transition process that prepares young people for all aspects of their lives, including health, lifestyle, daily living and future aspirations.

The network sought to raise the profile of transition care and improve the understanding and needs of young people and their families.
